APPLICANTS:
Within ten (10) days of date of application, applicant must submit the following to Fund Office:
1) Birth Certificate or Documentation Showing Proof of Age.
2) High School Diploma or GED Certificate
(Transcripts will be accepted if they show graduation date)
Foreign Transcripts - MUST BE TRANSLATED
3) Military DD214 - if applicable
PLEASE NOTE: CERTIFIED OR OFFICIAL COPIES NOT REQUIRED
